As you are likely aware, research has shown that older people without enough social connections have an increased risk of poorer mental health and well-being, which can impact their physical health and use of health services. This has become even more important in the current COVID-19 climate.
At Wentworth Healthcare, a NFP provider of the Nepean Blue Mountains Primary Health Network program, they support General Practices from Lithgow, the Blue Mountains, Penrith and Hawkesbury areas and have projects targetting identified community health needs.
They have believed for some time, that social isolation and loneliness of people in the area is a significant issue and have been working on a Social Connectedness Project since 2018. They have been striving to find ways for older people in our community to increase their social connections and reduce loneliness.
An online directory, MyHealthConnector.com.au has been developed as part of that project. The content was originally developed for the Hawkesbury and upper Blue Mountains region but it is available to services and community right across the Primary Health area from Lithgow to Penrith.
They are encouraging health service providers, support groups, and other community organisations who provide services for the community to add information about your service to this directory. It is free to use, they just ask that you keep your information as up-to-date as possible, which you can do yourself at any time by logging in to the website.
It would be great if you could add your service to the directory along with any groups/activities you run that you may feel are appropriate. It's an easy process, easy to update any changes to your service and it would be great for General Practice, other services and the community who use the directory to know what you are offering.
